Trailblazer Health Check Project in Conjunction with North Yorkshire Council
This is an introduction to the Trailblazer Community Health Check Project; a new initiative delivered in partnership with North Yorkshire Council and the Department for Work and Pensions.
The programme is designed to help local businesses support employee health and wellbeing — keeping people healthy, productive, and in work.
As part of this initiative, we are offering free on-site Cardiovascular Health Checks for employees within small and medium-sized businesses (under 250 employees) across North Yorkshire.
Each Health Check includes:
A 20-minute one-to-one appointment with an NHS Healthcare Assistant
Assessment of cardiovascular risk factors (blood pressure, BMI, cholesterol)
A lifestyle discussion covering smoking, alcohol, physical activity, and family history
Personalised health advice and signposting to further support if required
These checks are intended to support the early detection and prevention of heart disease, one of the leading causes of death in the UK. The programme is available to staff aged 25–64 years who have not received an NHS Health Check in the past five years and are not currently diagnosed with cardiovascular disease.
By identifying risk factors early, employers can make a meaningful impact on the long-term health of their workforce, helping to reduce sickness absence and the associated challenges, particularly in smaller organisations.
All checks will be delivered on-site by the Yorkshire Health Network, the General Practice Federation based in Harrogate, working as an extension of local GP practices.
Please note that, as part of this service, staff will be asked to provide consent for the healthcare professional to access and update their NHS medical record with the check results. All information is handled with strict confidentiality and is used solely for clinical care purposes.
